Student Preceptorship Program Instructors

The following criteria must be completed prior to accepting a student for a preceptorship program at Riverview Medical Center.
1. CONTRACTS:
Riverview Medical Center must have a current signed “contract” with your school/facility.
- If your school/facility does not have a current signed contract with Riverview Medical Center, FAX a copy of your schools contract to (715)424-9028 (ATTN Human Resources).
- The contract will be reviewed, signed, and returned within 1-2 weeks unless changes need to be discussed with the school/facility representative.
- A copy will remain on file at Riverview Medical Center.
- Riverview can NOT accept any students unless a current signed contract is on file.
2. CRIMINAL BACKGROUND CHECK:
Riverview Medical Center requires a current criminal background check (within 1 year) to be completed prior to being accepted into the preceptorship program.
- All students will be required to complete the on line criminal background check as part of the application process.
- Riverview can NOT accept any students unless a current criminal background check has been approved. A copy will remain on file in Human Resources.
3. PROGRAM DEFINITION:
The definition of a “student preceptorship program,” as defined by Riverview Medical Center has been placed on Riverview Hospital Association’s website for your review.
4. PROGRAM GUIDELINES:
A copy of the program guidelines has been placed on Riverview Medical Center’s website for your review.
5. STUDENT INSTRUCTIONS:
- The student is to complete and submit the online application which includes the health screening record and criminal background check questions.
- The student is responsible for FAXING and CONFIRMING with Employee Health that a signed clinic copy or school approved document of their current immunizations has been received and approved by Employee Health. Fax# (715) 424-9013 Office #: (715)-421-7441
