Student Preceptorship Program Students

Preceptor Home - Instructors - Students - Guidelines - Evaluation

Application process:

Complete the electronic application (which includes a non-employee health screening record and criminal background check information) and submit to Riverview Medical Center.  

Immunizations:

The following Iimmunizations or titers are required:

MMR vaccine X2, varicella vaccine X2 OR  titer proof of immunity (blood draw) to measles, mumps, rubella, varicella, and Hepatitis B.  If no titer proof of Hepatitis B or if titer shows negative, non-reactive, or Not immune, a signed Hepatitis B declination form must be on file.

  • A physician /clinic office copy of  ALL of the above immunizations or titers are to be attached to the health screening record and provided to Employee Health.   Fax # 715-424-9013.
  • PPD’s will be good for 1 years time up to the last date of your preceptorship experience.  I.E.. If the last day of your preceptorship is September 17th, 2009, and your PPD will not expire before this date, you will not need to have a repeat PPD done prior to starting your preceptorship experience.  IF your PPD will be expired prior to your last day of your preceptorship, you will need to have a repeat PPD done and fax a copy of the results to employee health.
  • Riverview will NOT process your application until ALL immunization records have been received via FAX by Employee Health.

Reminder:

Please have your instructor complete their portion of the process through the Riverview website (www.rhahealthcare.org).

Confirmation:

Once the required information has been received, the student will receive an electronic e-mail stating whether or not RHA has accepted or declined the student preceptorship proram application. We will evaluate your application but can not guarantee we will be able to fill all preceptorship requests.
